Financial Application Form
The Aggeus Foundation helps the forgotten patients in long term care by providing financial resources and support for treatment of their medical care.
Facility Administrators, Medical Directors, Directors of Nursing, and Social Service Directors, may apply on behalf of the residents under their care or supervision.
A committee will carefully review all requests for support from entities such as Nursing Homes, Assisted Living Facilities, CBRF’s, etc., as well as, individual requests.
All information must be completed and submitted at the same time for consideration.
While the Board of Directors of the Aggeus Foundation would like to support all those in need and grant financial aid, our program at this time is limited, though growing, in resources. It is limited to available resources and may be discontinued or revised at any time.
